Births Death and Marriages

Death certificates are issued by Births, Deaths and Marriages.

A death certificate that includes the cause of death is issued once the cause of death has been registered. A death certificate without the cause of death may be issued earlier; however, some organisations may not accept this version.

The death certificate is generally issued to the person who arranged the funeral, as the funeral director submits the relevant documentation to Births, Deaths and Marriages.

If you need to apply for a death certificate yourself, you will be required to provide proof of identity and pay the applicable fee.

The contact centre is open 8am to 4pm Monday to Friday excluding public holidays.
